I decided to poke around for IRS 1023 help, and found the following sites.

I'm sharing them with the startup list in the hope that it helps the
other FG startups.  I'm including FGCMH on this in case anyone is
looking over our application and wants to help (hint hint!).

terribly in-depth, but helpful nonetheless.

A $37 e-book with detailed information about the new 2006 IRS 1023 form.
 I'm tempted to buy this.  If I do, I'd happily share it with other
startups when they need it.

Here's a 1023 from 2004, with attached schedules available for review.
The form is different now, but I found it helpful to see a filled-out
form plus attachments.
